If you get health coverage through Medicaid expansion, expect an official letter from your state this summer. The rules attached to it take effect January 1, 2027.

What You Should Know

Under the One Big Beautiful Bill Act, states with Medicaid expansion are required to notify all affected enrollees about new work requirements before August 31, 2026, according to the Center for Health Care Strategies. Starting January 1, 2027, adults ages 19 to 64 covered through Medicaid expansion must document at least 80 hours per month of work, school, job training, or community service to keep their coverage. Failure to document compliance or qualify for an exemption results in loss of Medicaid.

The rule covers approximately 20 million adults in Medicaid expansion states, according to Stateline reporting from June 29. Nebraska became the first state to implement the new requirements ahead of schedule, starting May 1, 2026.

What Happens Next
