Medicare’s standard Part D drug deductible is rising from $615 to $700 in 2027 - an $85 increase, or 13.8 percent, the largest jump in years. For the roughly 50 million Americans enrolled in Medicare prescription drug coverage, that means paying the first $700 in annual drug costs out of pocket before the plan begins sharing costs. It takes effect January 1, 2027, and was locked in by CMS in April.
What Changes January 1
The $700 deductible applies to the standard defined benefit. Not every Part D plan charges the full deductible - some plans waive it for lower-tier generic drugs - but plans that do apply the standard structure will charge the full $700 before coverage begins. The deductible increase is steeper than the year before: in 2026, the deductible rose from $590 to $615, a $25 increase.
On the positive side, Medicare’s annual out-of-pocket spending cap on covered drugs also rises in 2027, from $2,100 to $2,400. Once you hit $2,400, your covered medications cost nothing for the rest of the year. Insulin stays capped at $35 per month regardless of where you are in the deductible cycle.
Who Pays More - and How Much More
For beneficiaries who take expensive brand-name medications at the start of the year, the $85 jump is directly visible. If you fill an expensive prescription in January and hit your deductible in the first month, you will pay $700 instead of $615 before the plan covers a dollar. For beneficiaries who primarily take generic medications that many plans exempt from the deductible, the increase may be less immediate.
Low-income subsidy recipients face their own adjustment: generic copays rise from $5.10 to $5.80 per fill in 2027, and brand-name copays go from $12.65 to $14.40. For beneficiaries on multiple brand-name medications, the per-prescription increase compounds.
The Annual Enrollment Period to Watch
The deductible and cap changes are already locked in through the CMS Contract Year 2027 Rate Announcement published April 6, 2026. The Annual Enrollment Period - October 15 through December 7 - is the window to compare plans and potentially switch to one with a lower or waived deductible structure. The current plan’s 2027 premium and formulary may also change. CMS and Medicare.gov will have 2027 plan information available starting October 1.
