The Social Security Administration announced a 2.8 percent cost-of-living adjustment for 2026 - adding about $56 per month to the average retirement benefit. For the roughly 50 million Americans enrolled in both Social Security and Medicare Part B, the Part B premium is deducted directly from the monthly benefit. When premiums rise faster than the COLA, the gap reduces what actually lands in the bank. In 2026, the math worked out to $56 added, $17.90 taken back - leaving a net increase of $38.10 for a beneficiary receiving the average benefit. That is a real gain, but it is 32 percent less than the headline number.

The 9.7 percent Part B increase in 2026 was the second-largest dollar increase in the program’s history, according to independent analyst Mary Johnson. It dwarfed the 2.8 percent COLA by a ratio of more than three to one.

Who Gets Hit Hardest and Why

The automatic deduction mechanism means the gap falls uniformly on everyone enrolled in both programs. For a beneficiary living on a fixed income close to the average benefit, a $17.90 monthly reduction is equivalent to losing one trip to the pharmacy, one utility bill installment, or several days of grocery spending.

Higher-income enrollees face a compounding effect. Those who pay IRMAA surcharges - the income-related premium adjustments that apply above $109,000 for single filers - saw the IRMAA amounts rise in 2026 as well.

What 2027 Projects

The gap is expected to narrow in 2027. Social Security’s COLA is currently forecast at 3.7 to 3.8 percent, while Part B is projected to rise roughly 3.25 percent. If those projections hold, it would be the first year since 2024 that the raise outpaces the premium increase. Neither number is confirmed until October (COLA) and November (Part B).
